Can a Catholic be forgiven by Jesus alone through prayer?

If you are truly sorry for the right reasons, you can make a perfect contrition. Since it is obviously very difficult to unbiasedly evaluate your own contrition, it is still typical to make a confession after a mortal sin (better safe than sorry over matters of eternal consequences being the reasoning) before receiving communion, or if in/facing danger.
